Required Enrollment Forms
The Enrollment and Reenrollment process is completed online through our Enrollment and Forms Portal (see link on right) by completing the following required forms:
- Registration Confirmation
- Enrollment Agreement (new students only)
- Student Vital Information Form
- Primary Parent Vital Information Form
- Co-Parent Vital Information Form (where necessary)
- Grandparent Vital Information Form
Instructions for accessing the Enrollment and Forms Portal will be emailed to families on the dates listed below or you can follow the "Step-by-Step Instructions" link at the right. Forms will be available through the Enrollment and Forms Portal beginning on the following dates:
