Reminder: Health Forms Are Due

Because the health and wellbeing of our students is a top priority, Stevenson partners with Magnus Health, an online provider, to collect, manage, and securely store student health information. Reminders are being sent every two weeks directly from Magnus Health indicating which health forms need to be completed for your child. Please pay attention to these reminders, as the completed forms are necessary for your student to begin school in just under two weeks.

To access your forms and begin completing them, you can connect to the Magnus Health portal here.

Below is some important information regarding health insurance and immunizations:

Insurance Requirement
For international students, proof of US-based insurance was due on July 1, 2019. If you did not provide this documentation, you will automatically be enrolled in a UnitedHealthcare (UHC) insurance plan elected by Stevenson. For more information regarding the insurance plan, please click here.

For international students who are enrolled in health insurance purchased through Stevenson, student insurance cards will arrive at the health center in August. We will upload the information to the Magnus Health portal on your behalf and you will be billed in the fall. Please note: Due to limitations within the Magnus Health system, you will continue to receive the reminder emails until the health center receives the insurance card from UHC and uploads it for you in August. Students will be reminded to retrieve the cards at the health center when they return in August.

Immunization Information
Several immunizations are required for your student to start school in August. For information about required immunizations, click here. For international boarding students who are unable to access vaccines abroad, please plan to arrive at school a few days in advance to receive the vaccine(s) before the start of school. The health center can provide you a list of local clinics in the area that provide vaccinations.

Thank you in advance for your prompt attention to your child’s health forms. Please contact the Health Center with any questions at
